The day of your first appointment, you can save a lot of time by downloading the forms to the right. Fill out the Patient Information form and both HIPAA forms before you arrive, and you’ll be able to breeze through our reception area.

Newton Cosmetic Dentistry does whatever it takes to perfect your smile. We accept all insurance policies, but we’re not a participating provider on any insurance company list.
